Our admission staff are located at Reception which is located at the front entrance to the Hospital. Upon admission you will be requested to complete a patient admission form giving your personal details and medical history.

What do I bring to Hospital?

• Medical Necessities
  • Current medications in the chemist's original labelled containers, and repeat prescriptions (if applicable)
  • Private health insurance details
  • Medicare card
  • Pension card (if applicable)
  • Pharmacy Entitlement Card (if applicable)
  • Veterans Affairs Card (if applicable)
  • Recent relevant X-Rays (if applicable)
    
• Personal Necessities
  • Dressing Gown and sleepwear
  • Slippers
  • Toiletries

If you are having a day procedure please ensure that you wear loose comfortable clothing. You may also like to bring a pair of slippers for comfort.

Please do not bring:

• Large sums of money.
• Jewellery
• Items of sentimental value that cannot be replaced
• Mobile telephones
